Personal information collected, viewed and retained may include name, birthdate or age, gender, family status, occupation, phone number, address and in some cases, salary/earnings and basic health questions when necessary. In specific cases, such as life and disability related insurance applications or claims, we and/or the insurance companies may ask for additional information from the insurance industry’s MIB Group, Inc. (Medical Information Bureau), health insurance companies, licensed physicians and healthcare professionals or related institutions in order to help administer or determine your eligibility for coverage.
When you make a claim for benefits that requires our assistance, we may also collect or view information to determine the eligibility of the claim, that may include the nature and type of expense.
COLLECTING WEBSITE DATA:
We and the web hosting company may collect information from our website visitors to improve the navigation, performance and overall user experience, and to evaluate the operation and use of our website. No personal information such as name, address, phone number etc. is collected by the web hosting company for this purpose. No third party is authorized nor should be asking you for any personal information on our behalf.
PROTECTING YOUR PERSONAL INFORMATION:
We carry out reasonable practices to ensure your personal information is protected from unauthorized access by implementing password requirements on computers and cell phones
that stores personal information. Paper forms with personal information are securely stored and are shredded before disposal.
